Upon joining pensby high school for girls or boys you must get this form signed by your parent/carer. This then gives the school permission to use photographs/videos of you for their advertising or promotion purposes.
When I filmed my piece, because the people were not eighteen, I had to make sure that they had one of these forms signed. As I knew the students they also gave me their personal consent of taking part in the film.
